4. Describe one recent urological procedure or service responsibility. State the date or month, your personal responsibility and supervision. Do not include patient-identifying information.
5. How do you plan escalation when a procedure or complication needs support outside the available facility?
6. Which endoscopic, imaging or surgical systems have you actually used, and for which procedures?
7. Can you type digital prescriptions with diagnosis and medicines for every patient you treat and forward them to Reception? Which systems have you used?
8. Describe one responsibility you held in clinical audit, quality improvement, infection control or NABH activity.
