2. What was your usual patient load and duty responsibility? State the consultant backup and supervision available.
3. Rate your experience for each task below.
4. Describe one recent ward, OPD or emergency-support responsibility. State the date or month, your personal responsibility and supervision. Do not include patient-identifying information.
5. How would you seek senior review for a patient or procedure beyond your competence?
6. List completed clinical training and procedures you have actually performed. Distinguish supervised and independent work.
7. Can you type digital prescriptions with diagnosis and medicines for every patient you treat and forward them to Reception? Which systems have you used?
8. Describe one responsibility you held in clinical audit, quality improvement, infection control or NABH activity.
