After your contact details and qualifications, you'll answer these 8 role questions:

1. Which areas best describe your recent practice? State the months worked in each.
2. List your main procedures and approximate numbers in the last 12 months. Separate primary operator, assisted and supervised cases.
3. Rate your experience for each task below.
4. Describe one recent operative or perioperative responsibility. State the date or month, your personal responsibility and supervision. Do not include patient-identifying information.
5. How do you arrange escalation and handover when a postoperative patient needs further specialist or critical-care support?
6. Which open/laparoscopic procedures and equipment are you comfortable with, and where would you require support?
7. Can you type digital prescriptions with diagnosis and medicines for every patient you treat and forward them to Reception? Which systems have you used?
8. Describe one responsibility you held in clinical audit, quality improvement, infection control or NABH activity.
